Primary Responsibilities: 

  • Assess patient’s health status and recommend care based on clinical judgment and protocols
  • Identify potential care and/or provider gaps
  • Coach consumers on treatment alternatives
  • Coordinate services and referrals to health programs and community services
  • Assesses and triage immediate health concerns

Required Qualifications: 

  • Active, Unrestricted Compact RN License
  • Ability to obtain additional state licensure as needed
  • 3+ years of clinical experience as a Registered Nurse
  • Proficient level of experience with Microsoft Office and Outlook applications
  • Designated work space and access to install high speed internet via cable/DSL in home

Preferred Qualifications:  

  • Experience working with Geriatric population
  • Experience in Case Management, Triage, ICU, Med/ Surg, Home Health or Long Term Care
  • Experience with Medicare and Medicaid population
