Job Summary:


Responsible for the development, planning, implementation, coordination, and evaluation/improvement of multiple programs including the Skilled Nursing Facility (SNF) Program, Inpatient Acute Rehab (IPR), Long Term Acute Care Hospitals (LTACH) and Intermediate Care Facilities (ICF).  Initiates and develops new models of service to improve and integrate patient care and service/programs with strategic, organizational, and business goals and objectives.  Ensures cost effective operations with maximum efficiency while maintaining high level of quality and safe care and services throughout the Region. Plans, directs, manages, develops, implements, and evaluates placement desk staff and SNF care coordination which may include utilization, processes, and clinical care to enhance admission, discharge/transfer to most timely, appropriate setting. Promotes continuity of care and work with SNFs to ensure high quality and care, utilization, administration of the Health Plan benefit and ensures that the program meets regulatory, accreditation, licensure and certification requirements. Develops, implements and maintains clinical practice standards and alternative care options to prevent admissions to acute hospital. Collaborates with clinical providers to ensure provision of high quality of care at contracted facilities. Participates in UM strategic planning at the National and Regional levels.  Development and oversight of the SNF and IPR quality programs.

Essential Responsibilities:


  • Accountable for the role modeling principles defined by the Care Experience standards, the KP Mission and any specific department/regional initiatives.
  • Establishes, implements and maintains patient care and service standards to meet members expectations.
  • Plans, develops, and evaluates  department strategies, processes, policies and procedures to improve systems and patient care outcomes/results across the continuum of care. Ensures consistent benefit interpretation and compliance with regulations. Establishes and sustains strategic relationships with key stakeholders.
  • Ensures compliance in meeting all Federal and State regulations and standards related to SNF, i.e. CMS, DOH, Life Safety,OSHA and TJC.
  • Provides leadership through direct and indirect reports to ensure member focused high quality, accessible, cost effective care, including interviews, selection, orientation, evaluation, coaching, discipline, performance feedback, rewards/recognition and staff accountabilities. Pursues professional growth and provides developmental opportunities, reviews and implements education based on the needs of staff with peers/staff/educators. Facilitates ongoing staff development and ensures competent staff.   Provides open feedback and coaching to drive performance improvement.
  • Directs and coordinates team efforts to optimize skilled days, including effective admission determinations, discharge planning, utilization of  services including ancillary services and timely placement.
  • Participates in utilization and multidisciplinary rounds for timely, appropriate medical treatment. Evaluates plan of care. Communicates admission and discharges to administrative staff and others as appropriate.
  • Assists in development of and monitors and controls payroll and non-payroll budget for LTC area to provide efficient, cost-effective patient services. Manages the overall regional budget ensuring spending within budget allocation, monitors and controls expenditures and provides input and direction to the budget planning process.
  • Oversees denials, termination of benefits, and appeals processes.
  • Coordinates statistical data. Presents reports as scheduled and as necessary.
  • Responsible for regional program operations to achieve business and organizational strategic objectives. Develops and implements business plans into tactical action items, aligning team and key stakeholder efforts. Ensures ongoing effectiveness of quality management program for the department to include selection of key processes and indicators of quality, monitoring, and evaluating improvement.
  • Establishes optimum interdisciplinary working relationships with chiefs of service, physicians, Continuing Care Coordinators, leadership and staff of  SNF, ICF and IPR facilities.
  • Ensures appropriate utilization of resources and provision of quality care to Health Plan members in SNF, IPR and ICF facilities. Oversight of nursing professional practice standards, KP standards of care and clinical competency of nursing staff in accordance with licensure, professional practice standards and regulatory requirements.  Oversight of clerical staff, ensuring appropriate benefit interpretation and statistical data. Collaborates with Medical Director to contract with community providers, SNF and IPR facilities and ancillary services.
  • Serves as primary Health Plan partner for physician leadership.
  • Liaisons with external review organizations. Researches questionable requirements which affect procedures. Updates staff on current changes in requirements by intermediaries.
  • May perform care coordination and case management to the extent necessary to ensure safe and timely transition from the acute hospital or clinic to SNF and IPR.
  • Maintain departments education records of training conducted, attendance, and clinical performance for nursing staff.
  • Ensures, with assistance, that licensures, certifications, registrations (LCRs) and other required documents are tracked and up-to-date for nursing staff.
  • Communicates goals, objectives, accountabilities, priorities, and authority parameters to assigned staff.
Basic Qualifications:
  • Minimum three (3) years of general management experience in a complex, multi-faceted healthcare setting and working with physician groups.
  • Minimum two (2) years financial management, budget development, program development, quality improvement or project management experience.
  • Minimum two (2) years Home Health/Hospice, Acute Rehab, Skilled Nursing or Health Plan operations experience.
  • Bachelors degree in nursing or Gerontology, Health Service Administration or related field OR six (6) years of experience in directly related field.
  • High School Diploma or General Education Development (GED) required.
License, Certification, Registration
  • Physical Therapist License (Oregon) OR Occupational Therapist License (Oregon) OR Registered Nurse License (Oregon)
  • Occupational Therapist License (Washington) OR Physical Therapist License (Washington) OR Registered Nurse License (Washington)
  • Drivers License (in location where applicable)
  • Basic Life Support within 2 months of hire
Additional Requirements:
  • Knowledge and understanding of SNF regulatory requirements.
  • Demonstrated knowledge of and skill in group presentations, creativity/innovation, customer service, group process facilitation, influence, interpersonal relations, oral communication, problem solving, quality management, and written communication.
  • Demonstrated knowledge of and skill in word processing, spreadsheet, and database PC applications.
  • Thorough knowledge of continuous quality improvement and its application in the healthcare setting.
  • Thorough knowledge of Medicare regulations/standards governing skilled nursing and acute rehab facilities.
  • Proven leadership skills
  • Excellent written and verbal communication skills, analytical problem-solving skills, and project management skills.
  • Experience working in a Labor/Management Partnership environment.
Preferred Qualifications:
  • Minimum two (2) years of change management experience in a healthcare setting
  • Minimum two (2) years managerial experience within Kaiser Permanente or similar Health Maintenance Organization
  • Previous recent RN nurse leadership experience in a primary care setting
  • Previous experience within Quality/Utilization Management
  • Demonstrated understanding of Nurse Practice Act, Labor Law, all related contracts, workmens comp, HR policies and procedures, OSHA, Employee benefits, National Compliance, Labor Management Partnership
  • Masters degree in Nursing, Health Care Administration or Business Administration or related field.
