Job Introduction

Maximus is currently seeking a Field Regional Nurse Manager to provide outstanding home assessment services to vulnerable populations and meet position qualifications to support the New York Independent Assessor Program (NYIAP). The program serves the State of New York by conducting a UAS assessment to determine eligibility for community-based long-term services. NYIAP Nurse Assessors conduct conflict-free assessments of these individuals using the Community Health Assessment tool to determine whether they qualify for these services. In this role, the Regional Nurse Manager will have the opportunity to lead a team and conduct home assessments.

 

*This position is hybrid and will support a regional area in New York state with surges in demand. The position requires attending and conducting assessments in consumer homes or through a telehealth modality, depending on operational need.

 

This position will operate in one of the following regions:  Central NY; Finger Lakes, NY; Western, NY; Queens, NY; Manhattan, NY; Brooklyn, NY; Bronx, NY.

Education and Experience Requirements

Duties & Responsibilities:

  • Lead a team of nurse assessors, including hiring, training, coaching, providing support in person and on the phone and conducting performance reviews.
  • Ensure new team members are competent and feel supported.
  • Conduct visits with team members to demonstrate, educate, and evaluate competency.
  • Perform in-home, in-facility, or telehealth assessments that includes evaluation of individual’s health status, strengths, care needs, and preferences using the UAS-NY assessment tool within the prescribed timeframes of the assessments.
  • Maintain a comprehensive working knowledge of community resources, payor requirements, and network services for target population.
  • Establish and maintain positive relationships with consumers, caregivers, and facility staff.
  • Maintain updated knowledge of the UAS-NY assessment process.
  • Review consumer medical documentation and/or health referral forms, as relevant to the case.
  • Document any concerns, conflicting information, other issues that surface during the evaluation process.
  • Emphasize continuity of care, thus reducing or eliminating fragmentation, duplication, and gaps in the treatment plan.

 

Minimum Requirements:

  • 4+ years RN experience in-home care.
  • Prior experience conducting assessments using the UAS-NY Community assessment tool.
  • RN license in the state of New York.
  • Bachelors’ Degree in health, social services, or communications.
  • Ability to travel locally.
  • Reliable form of transportation may include ride-hailing apps.

 

Preferred Qualifications:

  • Minimum of 1 year of supervisory experience in a clinical setting.
  • MA degree in a health, social services, or communications field preferred.

Pay Transparency

Maximus compensation is based on various factors including but not limited to job location, a candidate’s education, training, experience, expected quality and quantity of work, required travel (if any), external market and internal value analysis including seniority and merit systems, as well as internal pay alignment. Annual salary is just one component of Maximus’s total compensation package. Other rewards may include short- and long-term incentives as well as program-specific awards. Compensation ranges may differ based on contract value but will be commensurate with job duties and relevant work experience.
