At the 24-Hour Nurse Line the Health Coach Consultant utilizes a collaborative process of assessment, planning, implementation and evaluation, to engage, educate, and promote/influence members decisions related to achieving and maintaining optimal health status. Requires an RN with unrestricted active license.

 

– The 24-Hour Nurse Line provides a telephone-based information and decision support program to eligible members. The Nurse will be responsible for the following:

– Facilitates effective health care decision-making.

– Improves the quality of care.

– Improves member satisfaction with their health care plan.

– Facilitates appropriate use of health care resources.

– Provides up-to-date health care information to help facilitate members’ understanding of their health problems and possible options.

– Provides assistance by clarifying available options for the member’s particular health problem or issue.

– Helps members actively and knowledgeably participate with their provider in their own health care decision-making.

– Promotes healthy lifestyles to help lower personal health risks, preventing illness.

– Awareness of the member’s available health and wellness programs, and makes referrals to these programs as appropriate.

 

Required Qualifications

– Must have active, current and unrestricted RN compact state licensure in state of residence

– 3+ years of clinical experience required, specifically in: Disease Management, Emergency Room, Critical Care, Medical/Surgical Care, Oncology

– 1 year proficiency in Microsoft Office Suite (including Outlook and Microsoft Word)

– Must be able to work an 8 hour day shift every other weekend and 4 holidays per year; with earliest start time of 9am EST hours

 

Preferred Qualifications

– Managed Care experience preferred.

– Previous coaching experience preferred

– Model candidates have telephone triage nursing experience

Education

Associate Degree required

BSN preferred

 

 

Pay Range

The typical pay range for this role is:

 

$54,095.60 – $116,800.00

 

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
