DaVita is looking for an NP Lead. The Lead, Nurse Practitioner (“NP Lead”) functions autonomously, yet in collaboration with all members of the healthcare team to coordinate and facilitate quality, cost-effective care while minimizing fragmentation of the healthcare delivery system for ESKD and/or CKD patients.

The NP Lead will focus on improving clinical outcomes, care delivery, goal setting, and onboarding new NPs. The NP Lead will join us in the Des Moines, IA market.

Position Description: NP Lead will train and mentor Nurse Practitioners, Support Nurse Practitioners to complete CHEs. When seeing patients, complete comprehensive office visits on the go- primary care focused visits (chair-side in dialysis clinic, in-person visits and telehealth). See and manage complex patients in a short amount of time- 30 minutes.

Location: Round and Travel within an assigned geography to support NPs (trainings, meetings, and shadowing) and patients as needed.

Home Office, High Speed Internet and Reliable Transportation Required.

Position Goal: To prevent hospitalizations. Drive innovation and CHE completion.

Position Details:

  • Complete Comprehensive Health Assessments (80%) of the time when seeing patients.
  • Train and mentor NPs. Lead meetings and trainings.
  • Focused on innovation, problem solving and change management.
  • Drive CHE completion, committed to quality and quantity.
  • Support NP engagement and partnerships with Nephrology Practices.
  • Monday- Friday schedule with the ability to accommodate Teammates, patients and nephrology partner’s availability. Must be flexible.

  • Current license to practice in the state of practice
  • Current Drug Enforcement Administration (DEA) license required or eligible to obtain within 90 days of hire
  • Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification
  • Minimum of two (2) years’ experience as a Nurse Practitioner (NP)
  • Preferred experience as a Family Nurse Practitioner (NP), Primary Care Nurse Practitioner (NP), or experience in Internal Medicine, Cardiology, Nephrology
  • Demonstrated proficiency in clinical assessment, diagnosis, planning, implementation, documentation, and evaluation of chronic patients required
  • Intermediate computer skills and proficiency in MS Word, Excel, and Outlook required
  • Must be willing and able to complete home visits as needed

Preferred Qualifications:

  • Experience as a Family Nurse Practitioner (FNP), Primary Care Nurse Practitioner (NP), or experience in Internal Medicine, Cardiology, Nephrology or Case Management
  • Spanish-speaking preferred in some markets
  • Experience in managing chronic diseases (CHF, diabetes, hypertension, diabetes)
  • Manage pop. Health (hospitalization)

As the NP Lead you will perform Comprehensive Health Evaluations during patient visits in dialysis clinics, hospitals, when necessary to provide service to identify and assist with patients’ needs including but not limited to medical, social, emotional and financial concerns through medical record review, consultation with patient, family and healthcare team members.

As the lead you will act as the clinical lead for a team of NPs; assist with onboarding new NP’s; complete ongoing training for NPs through clinical best practice exercises and provide ongoing support to other NPs.
