Sentara Health Plans is hiring a Manager, Integrated Case Manager/RN!
This is a Full Time position with day shift hours and great benefits!
Applicants must be located in Virginia with travel to office approx 1x quarter.
Job responsibilities:
- Responsible for managing and coordinating the patient care program. This ensures the patient’s progression through the care continuum in a manner that achieves designed clinical and financial outcomes.
- Participates in the system development and implementation of integration of care transitions along the continuum of care.
- Works in close collaboration with the multidisciplinary team, in all settings to include ambulatory, post – acute providers and health plans.
- Serves as mentor by role modeling a professional practice consistent with organizational goals, customer service, clinical effectiveness, and standards of care.
- Responsible for developing and managing the departmental budget, compliance to federal, state and regulatory requirements.
- Manage 3-6 direct reports and responsibilities will include oversight of transition of care and case management teams
Minimum Qualifications:
- Required: Education Required: N-4YR or 6-8 YR- RN-Bachelor’s Level Degree or Masters Level Degree; OR MSW or /Master Degree in Counseling– Licensed Clinical Social Worker (LCWS) Degree or Licensed Professional Counselor (LPC)
- Certifications (Must have either a RN – BSN or MSN; or a Licensed Clinical Social Worker Degree/MSW or Licensed Professional Counselor (LPC)
- Required: Case Management- 3 years; Leadership- 3 years
- For Care Management departments, specialty certification required within one year of eligibility (ACM, CCM, CCCTM, CMAC or RN-BC). For other service lines, certification based on specialty area required within one year of eligibility.
