Job Description

A bit more about this role: 

This position represents an amazing opportunity  for an ambitious nurse practitioner leader  ready to take on a leadership role in developing colleagues and improving the effectiveness of our clinical teams.  As a CAV NP Manager, you will be responsible for clinical and operational oversight of a team of CAV Nurse Practitioners within Devoted Medical. This position will also have direct clinical responsibilities in caring for our patients.  On a day-to-day basis you will work closely with the NPs on your team, NP managers on the NP leadership team, Devoted Medical Directors, and cross functionally with multiple Devoted teams. This is an amazing opportunity for professional growth within the organization. You will report directly to the Devoted Medical Director of Nurse Practitioners.

 

Responsibilities will include:

  • General oversight of clinical performance for the NPs on your team
  • Reviewing and acting on CAV program Looker data
  • Close collaboration with market medical directors and the CAV program medical director
  • First level resource for procedural and process guidance for your team
  • Monitor clinical resource allocation and execute changes to meet the visit needs of the practice
  • Schedule oversight to include management of time off and requests associated with time off, monitoring daily, weekly, and monthly schedule density, and all schedule changes impacting patients scheduling.
  • On boarding guidance for new providers in clinical and process development
  • Meet one on one at least every 2 weeks and as needed with team providers to coach and performance manage
  • Schedule and run a team meeting every 2 weeks in collaboration with your Medical Director
  • Shared responsibility in the monthly CAV meeting

 

Your duties will be a combination of Managerial and Clinical. These will be done in collaboration with your Medical Director and/or the Director of Nurse Practitioners. The amount of time spent on each will vary based on current Devoted Medical priorities, new team members in orientation, performance of Key Performance Indicators (KPIs), and PTO.

 

Management/Leadership duties (approximately 30% of your total time)

  • Completing chart audits for each team member
  • Goal Setting and coaching to achieve goals
  • Performance Review of program KPIs for each team member (Looker data)
  • Recruitment and interviewing new clinical staff
  • Clinical process improvement
  • Steward our member centric and open clinical culture to our members and colleagues

 

Clinical Managerial Duties  (approximately 70% of your total time)

  • Complete Ride Alongs and Shadow comprehensive assessment visits (CAVs) including comprehensive diagnosis/disease review, medication reconciliation, assessment for quality of care (Stars/Hedis) interventions,review of suggested HCCs,  as well as social and home health/DME needs.
  • Work closely with other members of the member’s care team including their PCP, specialists, and other Devoted team members including pharmacy, clinical nursing, and social work as well as interfacing with family members and caregivers in order to coordinate care for the member and deliver a collaborative care plan.
  • Utilize our home grown electronic health information system for visits while also providing feedback on how to improve the interface.
  • Completing visits with members on your schedule. This will be approximately one day per week but may flux during orientation, specific product needs, PTO, and growth. In some instances when appropriate and compliant you may also provide telehealth visits to members located in other geographies as well as potentially perform home based visits (drive to member’s home).

Desired skills and experience:

  • Devoted Medical Nurse Practitioner for at least one year
  • Three or more years working in clinical practice as a Nurse practitioner
  • Experience mentoring and/or managing other clinicians
  • Experience working in interdisciplinary teams
  • Fluid usage of the Google suite of products

 

Licensure and Certification:

  • An active and clear RN and APRN license in your home state,  as well as national APRN certification is required at time of hire and must be maintained while employed at Devoted Medical.

 

Salary Range: $135 – 140K annually
