If you are located in MD, you will have the flexibility to work remotely* as you take on some tough challenges

Primary Responsibilities:

• Provide telephonic care management to highly vulnerable Medicaid members
• Offer members health and disease education, and empower them to actively participate in their care
• Consult with members and connect them to community resources
• Collaborate with Managed Care Organizations, providers, and other community partners to ensure there are no gaps in care
• Use solid communication and organizational skills to educate members, and prevent costly gaps in service
• Present cases in weekly internal rounds and monthly rounds with state, community, and MCO partners
• Cover crisis line for 2 hours daily

Required Qualifications:

• Master’s degree behavioral health OR Bachelor’s Degree Nursing (BSN)
• An active, unrestricted State of Maryland professional license in one of the following:
  • Licensed Professional Clinical Counselor
  • Independent Marriage & Family Therapist
  • Licensed Independent Social Worker
  • Registered Nurse
• 2+ years of experience in a related mental health environment
• 1+ years of experience working with Medicaid population
• 1+ years of Community Mental Health experience
• Proficient computer skills and good working knowledge of Microsoft Word

Preferred Qualifications:

• Dual diagnosis experience with mental health and substance abuse
• Experience working in an environment that required coordination of benefits and utilization of multiple groups and resources for patients
• Experience working with Medicaid population
• Proven discharge Planning and care coordination
• Maryland resident however not necessary
