If you are located in MD, you will have the flexibility to work remotely* as you take on some tough challenges
Primary Responsibilities:
- Provide telephonic care management to highly vulnerable Medicaid members
- Offer members health and disease education, and empower them to actively participate in their care
- Consult with members and connect them to community resources
- Collaborate with Managed Care Organizations, providers, and other community partners to ensure there are no gaps in care
- Use solid communication and organizational skills to educate members, and prevent costly gaps in service
- Present cases in weekly internal rounds and monthly rounds with state, community, and MCO partners
- Cover crisis line for 2 hours daily

Required Qualifications:
- Master’s degree behavioral health OR Bachelor’s Degree Nursing (BSN)
- An active, unrestricted State of Maryland professional license in one of the following:
- Licensed Professional Clinical Counselor
- Independent Marriage & Family Therapist
- Licensed Independent Social Worker
- Registered Nurse
- 2+ years of experience in a related mental health environment
- 1+ years of experience working with Medicaid population
- 1+ years of Community Mental Health experience
- Proficient computer skills and good working knowledge of Microsoft Word
Preferred Qualifications:
- Dual diagnosis experience with mental health and substance abuse
- Experience working in an environment that required coordination of benefits and utilization of multiple groups and resources for patients
- Experience working with Medicaid population
- Proven discharge Planning and care coordination
- Maryland resident however not necessary
