Pay: per visit – VERY competitive

Schedules: Visits average 45min-1.5 hours. 20-30 visits a week available (can break up depending on availability)

Overview:

• Meets with new patients in their home to orient them about services, identify needs, and to suggest physicians and services.
• Sees hospital at home patients for laboratory testing, examination, diagnosis, and treatment of chronic and acute episodic illnesses and minor injuries. Refers to physician as needed. Writes prescriptions and performs office procedures within the scope of expertise, protocols, and available equipment. Makes house calls and telehealth visits as needed.
• Performs physical examinations.
• Conducts support groups in subjects related to physical and emotional health (i.e., challenge of change, dealing with loss, dealing with chronic illness as caregiver, etc.).
• Coordinates health promotional activities (providing printed information, routine testing, flu shots, mammograms, etc.).
• Acts as a resource person for decisions, planning, and policies affecting the health of patients.
• Maintains patient records. Compiles reports concerning care at patient’s home.
• Collaborates with staff to identify patients’ needs and to help develop plan to meet these needs.
• Follows patients in hospital at home as needed for continuity of care.
• Able to navigate technology for telehealth visit.
• Other duties as assigned.

Minimum Qualifications:

• Master’s Degree in Science of Nursing or Physician Assistant Studies or related required
• Minimum 3 years of advanced practice provider experience required
• Advanced Practice Registered Nurse License or Physician Assistant License in the state of practice
• Family Nurse Practitioner or Adult Gerontology or Acute Care Nurse Practitioner certification or Physician Assistant Certification from National Commission on Certification of Physician Assistants required
• American Red Cross or American Heart Association Basic Life Support Course (BLS or BLCLS) and Certification
• American Red Cross or American Heart Association Advanced Life Support Course (ALS or ACLS) and Certification
• Satisfactory references from employers and/or professional peers
• Satisfactory employment verification
• Satisfactory licensing verification
• Satisfactory education verification
• Satisfactory criminal background check
• Satisfactory drug screen
