SUMMARY:

Reporting to our Chief Medical Officer, the Nurse Practitioner is a key member of the clinical team and is responsible for providing routine care for patients enrolled in our wellness programs with a focus on weight loss & sexual health. The Nurse Practitioner will be responsible for handling patient appointments and care plans under the prescribing authority of our Physician Partners. The ideal candidate must exhibit an excellent work ethic, take the initiative to self-manage, be able to work independently in a busy team environment, and be able to multitask and adapt to change.

RESPONSIBILITIES:

  • Provide compassionate care to diverse patients via our HIPAA compliant Telehealth platform.
  • Perform standardized patient health assessment reviewing past medical history, current prescriptions, and any new health concerns.
  • Manage patient’s medical care as appropriate.
  • Manage medication therapy effectively.
  • Document all patient encounters per documentation standards.
  • Communicate effectively with the entire care team including the patient’s primary care provider.
  • Work collaboratively as part of the clinical team to ensure a best-in-class wellness program experience.
  • Provide effective patient education using the Teach Back technique.
  • Participate in clinical case conferences.
  • Maintain excellent punctuality and attendance during work hours.
  • Other duties as assigned.

REQUIRED EXPERIENCE/SKILLS/EDUCATION:

  • Current Medical License & State Board Certification
  • Proficiency in the use of Microsoft Word
  • Telehealth Experience Preferred
