This is a full time position with dayshift hours and great benefits!

Job responsibilities:

  • Implement population interventions to improve health, wellness and quality performance including evidence-based preventive services such as immunization, chronic disease preventive care, cancer screening, medication follow up, and wellness visits.
  • Implement provider-based interventions to improve health, wellness and quality performance including building relationships with provider clinical staff, facilitate the scheduling and delivery of evidence-based healthcare services, obtaining medical documentation to close care gaps.

 

Job requirements:

  • Required: Registered Nurse License (RN) – Compact/Multi-State License
  • Required: N-4YR – RN-Bachelor’s Level Degree
  • Required: Nursing- 3 years
  • Preferred: Case Management- 2 years
  • Preferred: Previous Population Health and MCO/Health Plan experience
