Primary Responsibilities:

  • Provide telephonic outreach to engage members discharged to home/community following an inpatient admission
  • Conduct telephonic clinical case management with engaged members for the 30-day transition of care period. Services include conducting post discharge assessments, medication review and reconciliation, identification of gaps and barriers in discharge plans and SDoH needs, member education, referrals and care coordination support
  • Serve as a point of contact for members, facilities, and providers in support of member needs and care coordination

This is high volume, customer service environment. You’ll need to be efficient, productive, and thorough dealing with our members over the phone. Strong computer and software navigation skills are critical. You should also be strongly patient-focused and adaptable to changes.

Required Qualifications:

  • Current, unrestricted RN compact license
  • 3+ years of experience in a hospital, acute-care or direct care setting
  • 1+ years of Case Management experience
  • Demonstrate a proficiency in computer skills to navigate a windows-based environment, including internet searches, program navigation, and the use of multiple screens and tabs
  • Ability to accommodate a home-office designated workspace with high-speed Internet
  • Reside in Alabama

Preferred Qualifications:

  • BSN
  • Case management experience or CCM certification
  • Experience or exposure to discharge planning, utilization review, concurrent review, or risk management
  • Experience in a telephonic role
  • A background in managed care
