Job Description
Prefers candidates residing in either Bergen or Passaic County NJ

Standard working hours Monday -Friday 8am -5pm EST.

RN Assessor is responsible for face to face visit in the filed assessing members in their place of residence to evaluate the medical needs for service implementation. Addressing requests for services such as adult medical daycare, pediatric medical daycare, personal care assistant, nursing facility custodial requests, Personal preference program and MLTSS.
15-20 assessments minimum per week. Assessments take into account information from various sources to address all conditions including co-morbid and multiple diagnoses that impact functionality. Assessments include the member’s level of work capacity and related restrictions/limitations. Using a holistic approach assess the need for a referral to clinical resources for assistance in determining functionality.
Consults with supervisor and others on team in overcoming barriers in meeting goals and objectives. Utilizes case management processes in compliance with regulatory and company policies and procedures.
Utilizes interviewing skills to ensure maximum member engagement and discern their health status and health needs based on key questions and conversation. The RN Assessor will also be expected to mentor new hires, once, a level of proficiency has been attained in their role.

Successful completion of the NJ Choice certification is a contingency of employment. As per NJ Division of Aging Services, Office of Community Choice Options guidance, an exam score of 80% or higher is required, along with successful completion of the State training modules, and field mentoring component.”

Pay Range
The typical pay range for this role is:
Minimum: 58,760
Maximum: 125,840

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.

Required Qualifications
Minimum 1 year of clinical experience, e.g., hospital setting, alternative care setting such as home health or ambulatory care required.

-Active and Unrestricted RN license in NJ required

-Willing and able to travel up to 75 -100% using your own vehicle to meet members face to face in Bergen and Passaic Counties of NJ and surrounding areas/counties.

– Must be comfortable completing Field based assessments of people of all different and socioeconomic status.

Education
Associated of Nursing Required. Bachelor’s Degree Preferred.
