About the role
What you’ll do
Hire, train, manage, and develop employees to ensure that a qualified staff exists to meet business objectives.
Assist in the ongoing development, implementation, and evaluation of the On Belay Care Management program that addresses the triple aims: improving patient experience, improving the health of populations, and reducing the per capita cost of health care.
Prepare formal answers to, and recommend the adjustment of, grievances and disciplinary matters.
Responsible for coordination of care across the continuum and population health management, for low, medium, and high-risk Medicare patients.
Interpret and manage the implementation of nursing philosophy and objectives, departmental policies, and procedures, and apply recognized standards of nursing care and practice.
Plan, select and devise care coordination operations, work methods, procedures, workflow and standards for quality and quantity of work, including staffing standards.
Participate in special projects that include but are not limited to inpatient utilization and relationship development with hospitals and Skilled Nursing Facilities.
Qualifications
Registered Nursing license required.
Five (5) years clinical experience required.
Minimum of three (3) years management experience preferred.
Experience managing care management or case management within a Medicare ACO or population health modeled system preferred.
Proven leadership experience that includes team building, motivating staff with enthusiasm and compassion, communicating skills across disciplines.
Ability to lead multi-disciplinary teams while expanding a care management program.
Strong communication, customer service and decision-making skills.
Knowledge of budgetary processes and financial principles.
Knowledge of labor laws, standards of care, and regulatory standards required .
Willingness to perform Care Manager duties as needed.
Background check required.

The pay range for this role is:
130,000 – 140,000 USD per year (Remote – United States)
