What you’ll do

• Hire, train, manage, and develop employees to ensure that a qualified staff exists to meet business objectives. 

• Assist in the ongoing development, implementation, and evaluation of the On Belay Care Management program that addresses the triple aims:  improving patient experience, improving the health of populations, and reducing the per capita cost of health care.

• Prepare formal answers to, and recommend the adjustment of, grievances and disciplinary matters.

• Responsible for coordination of care across the continuum and population health management, for low, medium, and high-risk Medicare patients.

• Interpret and manage the implementation of nursing philosophy and objectives, departmental policies, and procedures, and apply recognized standards of nursing care and practice. 

• Plan, select and devise care coordination operations, work methods, procedures, workflow and standards for quality and quantity of work, including staffing standards. 

• Participate in special projects that include but are not limited to inpatient utilization and relationship development with hospitals and Skilled Nursing Facilities.

Qualifications

• Registered Nursing license required. 

• Five (5) years clinical experience required.

• Minimum of three (3) years management experience preferred.  

• Experience managing care management or case management within a Medicare ACO or population health modeled system preferred.

• Proven leadership experience that includes team building, motivating staff with enthusiasm and compassion, communicating skills across disciplines.

• Ability to lead multi-disciplinary teams while expanding a care management program.

• Strong communication, customer service and decision-making skills.

• Knowledge of budgetary processes and financial principles.

• Knowledge of labor laws, standards of care, and regulatory standards required .

• Willingness to perform Care Manager duties as needed.

• Background check required.
