Responsibilities
- Lead strategic initiatives to improve patient care coordination and optimize resource utilization.
- Analyze complex clinical data to identify trends and recommend actionable solutions that enhance quality of care.
- Collaborate with cross-functional teams to design and implement innovative case management programs.
- Drive continuous improvement by monitoring outcomes and adjusting strategies to meet evolving patient needs.
- Serve as a trusted advisor, providing guidance and mentorship to team members to foster professional growth.
Required Qualifications
- Registered Nurse (RN) license in good standing. Possess license in state of residence.
- Minimum 5 years of experience in case management or related clinical role.
- Proven ability to analyze data and develop strategic care plans.
- Strong communication and collaboration skills.
Preferred Qualifications
- Bachelor’s degree in Nursing.
- Experience with healthcare analytics and population health management.
- Certification in Case Management (CCM or equivalent).
If you will be working at home occasionally or permanently, the internet connection must be obtained through a cable broadband or fiber optic internet service provider with speeds of at least 10Mbps download/5Mbps upload.
For this position, we anticipate offering an annual salary of 77,500 – 129,100 USD / yearly, depending on relevant factors, including experience and geographic location.
This role is also anticipated to be eligible to participate in an annual bonus plan.
