We are hiring a part-time Nurse Practitioner to conduct Medicare Annual Wellness Visits (AWVs) via telehealth for patients located in New York State.
This is a 100% remote role focused exclusively on preventive care (no problem-based visits). New graduates are welcome to apply
Requirements
● Active, unrestricted NY NP license
● Eligible to participate in federal healthcare programs (not excluded)
● Comfortable using EHR systems and telehealth platforms
● Strong documentation skills
Responsibilities
Conduct Medicare AWVs via telehealth
● Complete HRA, medication reconciliation, screening review
● Develop personalized prevention plans
● Document thoroughly and accurately
● Identify care gaps (follow-ups coordinated by operations team)
Preferred
● Medicare or AWV experience
● Multi-state licensure
● Telehealth experience
