Description

Position Summary:

The Post Acute Coordinator position will be responsible for a variety of clinical and administrative duties including updating and maintaining patient records, managing provider schedules at nursing facilities and ensuring serial compliance, facilitating specialty referrals, imaging needs, and medication authorizations as indicated, ordering supplies, communicating effectively with patients and family. This position will work closely with medical providers, outside facility nursing, administration, and admitting staff to ensure quality patient care. Coordination with the internal Xpress Wellness RCM team and operations will be critical.

Duties and Responsibilities:

  • Participate with leadership in developing, improving, and growing medical care in long-term facilities.
  • Assist in ensuring the accuracy and timeliness of patient data meets company goals.
  • Coordinate the implementation of new tools and resources with the medical care in long-term facilities team.
  • Communicate issues and concerns with the operations team.
  • Work with the RCM team to coordinate the flow of accurate patient information from the clinical team to the billing team.
  • Works with long-term care facilities to correct problems, gain efficiency, and coordinate the flow of information needed.
  • Assist with credentialing of providers by assisting with gathering needed information.
  • Traveling
  • Respects and acknowledges clients rights and maintains strict confidentiality in all client matters; Investigates and resolves incidents and issues as required while maintaining HIPAA compliance and associated relevant guidelines and regulations.
  • Pre-charts patient information with a high degree of accuracy, applying correct medical terminology and documentation standards to support efficient provider workflow.
  • Other Duties as assigned, this is a safety-sensitive position
Requirements

Qualifications:

  • Education:
  • High school degree required. Associate’s degree or higher preferred.
  •  Experience:
  • 3+ years of clinical experience in medical office or nursing facility preferred.
  •  Skills:
  • Strong communication, organizational, and problem-solving skills.
  • Proficiency with Microsoft office suite and electronic medical records.
  • Technically savvy and comfortable using tools such as mobile iPads for charting
  •  Licenses/Certifications:
  • CPR certification, Registered or certified Medical Assistant or alternate Certified Nurse Aid, or Certified Medication Aide preferred
