Role Overview
We’re seeking Primary Care NPs to provide on-site care to Assisted Living/RCFE residents across Stockton, Modesto, and Amador.
What You’ll Do
- Routine & urgent assessments; chronic disease management
- Post-discharge follow-ups, med reconciliation, labs/diagnostics
- Coordinate with facility staff, specialists, families/POAs
- Same-day documentation; CMS/regulatory compliance
Schedule & Setting
Full-time position with a structured weekday schedule. You’ll spend 3 days delivering on-site care across Stockton, Modesto, and Amador, with strategically clustered routes designed to minimize travel and maximize time with patients. The remaining 2 days are remote, giving you the flexibility to work from home for administrative and documentation tasks.
Requirements
Qualifications
- Active CA NP & RN licenses; active furnishing license; NPI, DEA, BLS
- Geriatrics or facility-based experience preferred; EMR proficiency
Benefits
Compensation
- Annual compensation range: $150,000–$175,000 (based on experience, schedule, and patient volume)
- Additional earning opportunities available based on coverage and panel needs
- W-2 with full benefits available; mileage reimbursed per policy
