Remote RN Case Manager (Costco Team)
📍 Remote (Preferred: San Antonio, TX area)
🕒 Full-Time | Monday–Friday | No Weekends or Holidays

Join our Costco team as a Remote RN Case Manager, where you’ll play a key role in helping members achieve better health outcomes through compassionate, coordinated care.
What You’ll Do
- Conduct telephonic (and occasional in-person) assessments to understand member needs
- Develop and manage personalized care plans to improve health and wellness
- Coordinate care across programs, providers, and resources
- Identify risks and remove barriers to care using clinical judgment
- Engage members through meaningful conversations to support their health goals
- Collaborate with a multidisciplinary team to optimize outcomes
Schedule
- Monday–Friday, 8:00 AM – 5:00 PM CST
- Occasional rotation to 10:00 AM – 7:00 PM CST
- No weekends or holidays
What You’ll Bring – REQUIRED
- Active, unrestricted multistate RN license in state of residence
- 3+ years of clinical nursing experience
- Strong computer skills (Microsoft Office + web-based systems)
- Ability to work the required schedule
- Associates Degree in Nursing
Preferred
- Case management experience (integrated model)
- Bilingual (Spanish/English)
- BSN
- Located within 45 minutes of San Antonio, TX (7034 Alamo Downs Pkwy)

Pay Range
The typical pay range for this role is:
$60,522.00 – $129,615.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
