On-Call Registered Nurse (7 On / 7 Off)
After-hours support. Mostly remote. Critical impact.
The On-Call Registered Nurse provides after-hours triage, clinical guidance, and urgent in-person visits as needed for hospice patients receiving care in home or facility settings, supporting comfort, safety, and continuity of care.
Schedule, Hours & Territory
• Dedicated on-call role with a rotating 7 days on / 7 days off schedule
• Coverage hours are 4:30 PM to 8:30 AM
• Assigned coverage area determined by branch needs and patient census

Key Responsibilities
• Triage patient and caregiver calls after hours
• Provide clinical guidance and symptom management
• Complete urgent in-person visits as needed
• Coordinate care with interdisciplinary team members
• Document all assessments, interventions, and communication accurately and timely in the EMR HCHB (Homecare Homebase)
Qualifications
PROFESSIONAL EXPERIENCE/QUALIFICATIONS:
- Help optimize the capital structure and support strategic initiatives.
- Maintains the agency’s mission, philosophy and core values.
- Ensures compliance with agency policies and procedures regarding operations/processes, including but not limited to those regarding patient care, patient complaints, incidents, safety and emergency management.
- Always maintains patient confidentiality, including all HIPAA regulations.
Regulatory Requirements
- Licensed or registered in the state in which he/she currently practices..
- Must pass a criminal background check & MVR check.
- Completed health statement acknowledging ability to perform the duties of the position.
- Valid state driver’s license.
- Proof of current automobile insurance.
- Current CPR card; On-line CPR certification accepted; Hands-on CPR certification preferred.
- TB testing per agency policy; (1 or 2 step TB skin test within 12 months of hire & annual TB symptom screening thereafter).
Industry experience
- Ability to work independently, make accurate, and at times, quick judgments. Ability to supervise others appropriately. Ability to respond appropriately to crisis outside of a hospital setting. Acceptance of and adaptability to different social, racial, cultural and religious modes. Completes Hospice training program.
- Active patient contact within past three years, preferred.
- Minimum 2 years of experience as a registered nurse, preferred
EDUCATION:
- Graduate from an accredited school of nursing
- Currently licensed as a registered nurse to practice in the state.
