Marin Post Acute is looking for a knowledgeable and detail-oriented MDS Coordinator (RN or LVN) to join our clinical leadership team. This is a key role focused on accuracy, compliance, and driving quality resident outcomes. Key Responsibilities
- Oversee and manage theย MDS/RAI processย in compliance with federal and California state regulations
- Completeย accurate MDS assessments, care plans, and supporting documentation
- Collaborate with theย interdisciplinary team (IDT)ย to ensure individualized, resident-centered care
- Monitor and improveย quality measures, reimbursement accuracy, and compliance standards
- Provideย training and guidanceย to nursing staff on MDS processes and updates
- Partner with leadership onย audits, reporting, and performance improvement initiatives
Qualifications
- Activeย California RN or LVN license
- Minimum 1 year of MDS/RAI experience preferred
- Strong knowledge ofย PDPM, care planning, and CMS regulations
- Exceptionalย attention to detail and organizational skills
- Ability to work collaboratively with clinical and administrative teams
