You will manage the resident assessment process to ensure the facility meets all healthcare guidelines. Your main goal is to make sure every patient has an accurate Minimum Data Set (MDS) assessment and a personalized Plan of Care.

Key Responsibilities

  • Assessment Management: Accurately complete and submit all required patient assessments (MDS/PPS) on time.
  • Financial & Data Oversight: Track PDPM (Patient Driven Payment Model) tools and analyze data to ensure the facility is operating efficiently and meeting quality standards.
  • Care Planning: Lead interdisciplinary teams to create, review, and update individualized care plans for residents.
  • Training & Supervision: Teach new MDS Coordinators and oversee the assessment staff.
  • Compliance: Ensure all documentation follows state and federal laws, maintaining strict patient confidentiality.

Requirements

  • Education: Degree from an accredited School of Nursing (BSN preferred).
  • License: Must have an active, valid Nursing License in good standing.
  • Experience: 3 years of supervisory experience and Long-Term Care (LTC) experience preferred.
  • Skills: Proficiency in Microsoft Office and experience with PointClickCare (PCC) software.

Working Conditions

  • Physical: Mostly office-based (sitting, typing, and talking), but may require standing, walking, and lifting up to 25 lbs. Some travel may be required.
  • Environment: Low to moderate noise; professional clinical setting.

Benefits

  • Health: Medical, dental, and vision insurance.
  • Financial: 401(k) plan and performance bonuses.
  • Balance: Generous Paid Time Off (PTO).
  • Growth: Paid training and clinical advancement opportunities.

We are an equal opportunity employer. All qualified applicants will receive consideration for employment without regard to race, color, religion, sex, sexual orientation, gender identity, national origin, age, disability, protected veteran status, or any other legally protected status.
