Nurse Case Management Lead Analyst
This role leads the PACE sleep therapy queue and supports day-to-day operations to promote efficient workflow, strong team performance, and high-quality service delivery. The position is responsible for reviewing work in progress, ensuring adequate queue coverage, providing coaching and guidance to staff, managing escalations, and assisting with data analysis to identify trends and improvement opportunities.
Responsibilities
- Lead the PACE team and support daily management operations of the sleep therapy queue.
- Review work in progress to ensure timely completion and alignment with team priorities.
- Monitor team workload and ensure adequate queue coverage to maintain service levels.
- Provide coaching, guidance, and day-to-day support to team members to encourage strong performance and professional development.
- Manage escalations by identifying issues, coordinating resolution, and communicating updates as needed.
- Assist with data analysis to identify workflow trends, gaps, and opportunities for operational improvement.
- Collaborate with leadership and cross-functional partners to support process consistency and efficient care coordination.
Qualifications
Required:
- Current RN license in good standing. License must be in state of primary residence.
- Minimum 2 years of clinical nursing experience, including 2 years in case management or care coordination or prior authorization.
- Minimum one year PACE processes and sleep medications experience.
- Ability to review active work, prioritize tasks, and support coverage across team queues.
- Strong analytical skills with the ability to interpret operational or healthcare data and apply insights to improve performance.
- Excellent communication, leadership, coaching, and problem-solving abilities.
- Ability to manage escalated issues in a professional, timely, and solution-oriented manner.
- Ability to manage multiple priorities in a fast-paced environment.
- Ability to multi-task effectively while maintaining accuracy and meeting deadlines.
Preferred:
- Bachelor’s degree in Nursing or related field.
- Experience with value-based care models and population health strategies.
- Proficiency in case management software and electronic health records (EHR).

For this position, we anticipate offering an annual salary of 77,500 – 129,100 USD / yearly, depending on relevant factors, including experience and geographic location.
This role is also anticipated to be eligible to participate in an annual bonus plan.
