The Role
We’re looking for a Registered Nurse (RN) – Virtual to provide remote clinical oversight, telephonic triage, and care coordination. In this 100% remote role, you will perform clinical assessments, monitor patient treatment plans, and collaborate with physicians and interdisciplinary teams to ensure quality care and regulatory compliance.
What You’ll Do
- Conduct virtual clinical intakes, health screenings, and wellness checks with patients and caregivers.
- Perform telephonic triage and clinical assessments for patient calls, directing care using established protocols.
- Review and update care plans in alignment with physician orders and state regulations.
- Monitor Remote Patient Monitoring (RPM) data and vital trends, flagging clinical changes for the care team.
- Serve as a primary virtual point of contact for patients, families, attending physicians, and home care staff.
- Maintain accurate, timely clinical documentation in our EHR platforms.
- Participate in virtual case conferences, team huddles, and quality assurance reviews.
What You Bring
- Active, unencumbered Registered Nurse (RN) license in the state of New Jersey required (Multi-state Compact Licensure preferred).
- Minimum of 1–2 years of clinical experience in home health, triage, or remote care management.
- Strong clinical triage, assessment, and critical-thinking skills.
- Professional, empathetic communication skills across phone and video channels.
- Proficiency with Google Workspace and EHR care management systems.
- Dedicated home office setup with high-speed internet meeting all HIPAA privacy standards.
