1.Identifies and assists with the needs of patients including, but not limited to, medical, social, emotional, and financial concerns through medical record review, consultation with patient, family, and healthcare team members 20%
2.Implement innovative nursing interventions appropriate for short and long-term needs of patients 15%
3.Ensure care coordination to complement and support medical treatment plans while integrated psychosocial, physiological, and discharge planning needs and post-discharge follow-up to prevent hospital readmission 15%
4.Coordinate healthcare services to members across continuum of care; provide follow-up for member care and evaluate outcomes 15%
5. Assist patients with identifying their goals, values, preferences and develops care plan that aligns with patient wishes 10%
6. Provide ongoing awareness and education of DaVita IKC services to providers and other teammates 5%
7. Assists pts potentially ready to transition from active to passive treatment/intervention, including educating patients on treatment options (i.e., hospice or palliative care) and connecting them with these resources 10%
8. Collaborate with and directs the day to day responsibilities of non-clinical teammates, as required (i.e. Integrated Care Assistant & Care Coordinator) 5%
9. Other duties as assigned. 5%
2-4 years experience. Associate's degree or apprenticeship.
A.N.D. degree or higher from accredited school of nursing or three year diploma from accredited diploma program Required
Minimum of two years working in palliative or hospice care setting Required
At least one year of experience in healthcare performance coaching or case management Preferred
At least two years of renal nursing experience Preferred
RN license in state(s) of practice Required
Certified Nephrology Nurse and/or Certified Case Manager Preferred
