Hospice Triage Nurse (Remote)

Key Responsibilities:
Answer all incoming calls and communications and provide telehealth assessments, pain management, and symptoms.
Assess the patient and family’s need for a home visit and scheduling through HCHB and notify the patient and family of the expected visit time.
Managing HCHB workflow including documentation of coordination notes and back office workflow.
Serves as an after-hours point of contact to ensure referrals are documented and ensure timely admissions.
Coordinate patient care and communication with interdisciplinary teams.
Key Qualifications:
Practical Nurse, Vocational Nurse, or Registered Nurse licensed in the state of employment
Certified Hospice and Palliative Nurse and/or ASN/BSN preferred
Minimum 3 years of hospice nursing experience
Remote access Wi-Fi and phone
Demonstrated expertise in pain management, symptom control, crisis intervention, and emotional and pre-bereavement support
Excellent critical thinking skills and clinical judgement
Previous experience of operating and documenting in Homecare Homebased
Excellent verbal and written communication skills
Self-directed and able to work with minimal direct supervision
Ability to work outside of posted regular business hours, including overnights and weekends
