Job Summary
The Telehealth Nurse Practitioner (NP) provides holistic, compassionate, and patient‑centered care to adult and geriatric Members through virtual visits, telephonic/video assessments, and remote chronic disease management. This full‑time role focuses on delivering high‑quality clinical care in a remote environment while collaborating with physicians, RNs, case managers, and interdisciplinary teams to diagnose, manage, and prevent acute and chronic conditions.
The NP is responsible for Plan Medical Management, virtual health risk assessments, chronic disease management, remote triage, and care coordination. This position requires strong communication skills, clinical judgment, and the ability to deliver value‑based care through telehealth platforms while maintaining compliance with state regulations, clinical protocols, and collaborative practice agreements.
Qualifications
Qualifications
• Minimum 2 years of experience as a Nurse Practitioner in adult, geriatric, or family practice settings
• Ability to perform comprehensive virtual assessments and manage chronic disease remotely
• Strong clinical judgment with experience supporting medically complex adult/geriatric populations
• Skilled in medication reconciliation and identifying gaps in care
• Graduate of an accredited NP program (Adult/Gerontology Primary Care preferred)
• Active RN and NP licensure in the state of practice; board certified (AANP or ANCC)
• Current CPR certification
• Highly organized, tech‑savvy, and comfortable using telehealth platforms
• Commitment to ongoing learning, feedback, and professional development
Preferred
• Geriatric certification (or willingness to obtain)
• Bilingual (English/Spanish)
• Familiarity with community resources and behavioral health needs
• Experience serving vulnerable or underserved populations
Essential Functions
• Perform virtual comprehensive health risk assessments for assigned Members
• Conduct telephonic and video‑based evaluations, including chronic disease management and acute symptom triage
• Review and interpret medical, surgical, and social histories through EMR and telehealth platforms
• Recommend and implement treatment plans, including prescribing medications, ordering tests, and providing wellness counseling
• Reconcile medications and ensure safe, appropriate use
• Identify quality improvement opportunities and develop individualized care plans
• Collaborate remotely with physicians, case managers, social workers, caregivers, and facility partners
• Provide virtual education to Members and caregivers regarding disease management and wellness
• Refer Members to PCPs, case management, or disease management programs as needed
• Maintain compliance with HIPAA, confidentiality standards, and PHI regulations in a home‑based work environment
• Diagnose conditions and create comprehensive, evidence‑based plans of care
• Achieve clinical performance targets and quality benchmarks
• Build positive rapport and engagement with Members and families through virtual communication
• Participate in committees related to clinical guidelines, quality improvement, and compliance
• Maintain accurate and timely EMR documentation
• Support remote care coordination workflows and integrated care team communication
