GENERAL SUMMARY

The Pathway Program Navigation Manager will be responsible to lead and coordinate the implementation and ongoing development of clinical pathways for Prostate Cancer (PCA) Bladder Cancer (BCA) Benign Prostatic Hyperplasia (BPH) and Overactive Bladder (OAB). The role will focus on education coordination, playbook implementation, driving quality improvement through data metrics and pathway adherence to optimize patient outcomes and program effectiveness. The Pathway Program Navigation Manager is pivotal in advancing standardized, evidence-based care for urologic conditions by ensuring pathway adherence and enhancing patient and provider education across multiple disease.

ESSENTIAL JOB FUNCTION/COMPETENCIES

  • Lead the development, implementation, standardization, and ongoing optimization of enterprise-wide clinical pathway and navigation programs across multiple affiliates and disease states.
  • Develop and deliver education and training programs for clinical navigation staff on PCA, BCA, BPH, OAB, and other approved clinical pathways to promote understanding, consistency, and adherence.
  • Provide leadership and oversight to nurse navigators and other clinical team members, ensuring compliance with established pathway standards, protocols, and best practices.
  • Collaborate with physicians, nursing leaders, pharmacy teams, and operational stakeholders to successfully integrate clinical pathways into daily practice.
  • Serve as the organization’s subject matter expert for clinical pathways and navigation programs, providing guidance and consultation to affiliated practices and multidisciplinary teams.
  • Monitor, collect, and analyze pathway adherence metrics, key performance indicators, and patient outcomes to evaluate program effectiveness and identify opportunities for improvement.
  • Identify barriers to pathway adoption and develop corrective action plans and process improvements to enhance compliance and patient outcomes.
  • Maintain and update clinical pathway content, educational materials, and implementation tools based on approved clinical guidelines, emerging evidence, and recommendations from the clinical board.
  • Prepare and present regular reports, dashboards, and program updates to executive leadership and key stakeholders regarding pathway performance, outcomes, and strategic initiatives.
  • Partner with clinical and operational leadership to drive quality improvement initiatives that support evidence-based care, operational excellence, and improved patient outcomes.
  • Foster collaboration among multidisciplinary teams and affiliates to promote consistency, knowledge sharing, and best practices across the organization.
  • Engage with external partners, industry organizations, and professional societies to remain current on emerging trends, best practices, and advancements in clinical pathway management.
  • Advocate for the resources, technology, and support necessary to sustain and expand navigation and clinical pathway programs across the enterprise.
  • Employees shall adhere to high standards of ethical conduct and will comply with and assist in complying with all applicable laws and regulations. This will include and not be limited to following the Solaris Health Code of Conduct and all Solaris Health and Affiliated Practice policies and procedures; maintaining the confidentiality of patients’ protected health information in compliance with the Health Insurance Portability and Accountability Act (HIPAA); immediately reporting any suspected concerns and/or violations to a supervisor and/or the Compliance Department; and the timely completion the Annual Compliance Training.
  • Performs other position related duties as assigned.

CERTIFICATIONS, LICENSURES OR REGISTRY REQUIREMENTS

  • RN licensure required.
  • BLS certification required.

KNOWLEDGE | SKILLS | ABILITIES

  • Knowledge of PCA, BCA BPH, and OAB management.
  • Proven experience in program coordination, education, and quality improvement initiatives.
  • Excellent leadership, communication, and interpersonal skills.
  • Ability to analyze data and translate findings into actionable improvement.
  • Familiarity with clinical pathway development and implementation processes.
  • Excellent verbal and written communication skills.
  • Excellent organizational skills and attention to detail.
  • Strong problem-solving skills.
  • Skill in using computer programs and applications including Microsoft Office. Knowledge in healthcare systems operations such as EMR.
  • Ability to coach and build skills within the team.

EDUCATION REQUIREMENTS

  • Graduate of an accredited nursing program required. BSN preferred.

EXPERIENCE REQUIREMENTS

  • Registered Nurse (RN) with clinical experience in Uro-oncology and/or oncology preferred.

REQUIRED TRAVEL

  • Ability to travel periodically to support affiliate engagement, program implementation, training, and ongoing performance initiatives.
