Description

POSITION SUMMARY:    Under the general supervision of the Center Director or RN Supervisor IV the Registered Nurse II is responsible for providing patient triage in person and telephonically. The Registered Nurse II will demonstrate that they are capable and competent in providing patient care and providing support for the providers, health centers, support staff and other site or program-based RNs. The Registered Nurse II will conduct patient triage, respond to patients concerns, provide education and consultation, review symptoms, vaccination counseling and education, provide lab results, participating in the ongoing education of support staff and collaborating with medical providers to ensure optimal health of our patient populations

Essential Functions
•    Ability to make quick decisions, multitask and handle stress; strong written and verbal communication skills; ability to work as part of a team; interpersonal skills; compassion.
•    Assists in planning and coordinating appropriate agency and community responses to disease outbreaks; participates on public health emergency response teams as needed.
•    Records patients medical histories, performs diagnostic tests, administers medications, and develops patient care plans in conjunction with the primary care team.
•    Conducts co-visits and assists providers during examinations and procedures, operating and monitoring medical equipment as needed.
•    Supports and enhances the patients’ physical and emotional stability; explains nature of illness, plan of care, procedures and treatments to patient, family, and significant others.
•    Performs nursing procedures and designated medical treatments within scope of authority; observes and assesses patients’ conditions and behavior; reports significant changes to physician.
•    In addition to assessing patients experiencing symptoms, the Registered Nurse II will need to triage the patient and relay their condition to the care team.
•    Participate in the coordinated care for patients seeking care via telephone and MyChart messages.
•    Follow up with patients and ensure communication with the primary care team.
•    Promotes independence by teaching patients and families to understand medical conditions, medications and self-care skills.
•    Presents patient and family teaching programs based on identified health care goals.
•    Coordinates with Care Transitions team for all hospitalized patients to ensures timely follow up and return to primary care.
•    Uses clinician-approved standardized procedures to order labs and preventive screenings, and to refill routine medications.
•    Coordinates care closely with other team members to identify patient self-care goals and to assure that medical and psychosocial needs are met. Facilitates follow up care as needed.
•    Acts as a resource for support staff.
•    Maintains absolute confidentiality of work-related issues, patient records, personnel records and County information; complies with the confidentiality standards of the Privacy Act of 1974, {U.S.C. § 552A} as amended, and HIPAA policies and procedures.

Qualifications

Qualifications
•    Current, unencumbered license to practice as a Registered Nurse in California.
•    Ability to effectively coordinate care with triage assistants and other support staff, encourage and nurture development and growth, to build a strong and productive team.
•    Strong organizational, administrative and problem-solving skills, and ability to be flexible and adaptive to change.
•    Ability to effectively present information to other team members.
•    Ability to seek direction/approval on essential matters, yet work independently with little onsite supervision, using professional judgment and diplomacy.
•    Work in a team-oriented environment with several professionals with different work styles and support needs.
•    Excellent interpersonal, verbal, and written skills.
•    Conduct oneself in internal and external settings in a way that reflects positively on LifeLong Medical Care as an organization of professional, confident and empathetic staff.

Job Requirements  
•    Associates or Bachelor’s Degree in Nursing.
•    Current and valid California Registered Nurse license and BLS.
•    Proficient in Microsoft office suite
•    Electronic health records, EPIC preferred.
•    A minimum of 1 year of experience as a LifeLong Medical Nurse or 2 Years of experience as a Registered Nurse.
Job Preferences
•    Ideally Bilingual in Spanish
•    Ideally holds a California Public Health certification.
