KEY RESPONSIBILITIES
- Monitor patient status remotely using advanced telehealth technology and electronic health record systems (Epic, Cerner, Meditech).
- Identify changes in patient condition and facilitate appropriate clinical escalation and intervention.
- Support bedside clinicians through real-time clinical consultation and workflow guidance.
- Coordinate communication among physicians, nurses, case managers, and other members of the care team.
- Assist with adherence to evidence-based practices, quality initiatives, and patient safety protocols.
- Document clinical activities and recommendations within designated electronic health record systems.
- Participate in multidisciplinary rounds and care coordination activities across partnered healthcare facilities.
- Serve as a clinical resource for hospitals, emergency departments, intensive care units, step-down units, post-acute settings, and home-based care programs.
REQUIREMENTS
- Active unencumbered Compact RN license
- Active NY license required
- Current BLS and ACLS certifications required.
- Minimum five (5) years of acute care nursing experience.
- Experience in ICU, step-down, emergency, or progressive care environments strongly preferred.
- Previous experience as a Charge Nurse, Preceptor, Team Lead, Clinical Coordinator, or similar leadership role preferred.
- Experience utilizing electronic medical record systems such as Epic, Cerner, or Meditech.
- Comfortable working in a fast-paced telehealth and technology-driven environment.
